Register at Rushey Green Group Practice

Our list is OPEN to new patients!

We register all patients that live within our inner boundary area. The roads within our inner boundary are listed in our catchment area. The whole Borough of Lewisham is our outer boundary area.

There are two ways that you can register with the Practice. Which ever way you choose you will need to come into the Practice with 2 forms of ID in order to be registered.

1. Register in Person

To register in person you will need to come in to pick up a copy of our registration form or print out and bring it to reception.

Complete the form and return it to us along with 2 forms of ID. A photograph ID (e.g. passport / drivers licence) AND a proof of address (e.g. utility bill / bank statement).

Registering Adults

Adult Registration Form

Anyone aged 16 or over is deemed an adult.

Upon viewing your two forms of ID you will be given a new patient pack which contains plenty of information about the Practice and all the services that we provide.

It will take us approx 5 days to manually input all your data onto our system after which time you are able to book an appointment to see a doctor or nurse when necessary. We will also ask you to book an appointment with our Health Care Assistant for a New Patient Check.

If you register online the registration process will be much faster.

Registering Children

Children Registration form under 16 (Click here)

We can only register children if a parent or guardian is registered with the Practice or is registering at the same time. We will need to see the following:

Parents Proof of ID (e.g. passport)

A copy of the child's Birth Certificate showing the parent's names

The child's Red Book or a record of any immunisations they have had.

Temporary registration

If you are in the area for no more than 6 months we are also able to provide you with temporary registration.

Re-registering

If you have been a patient of this Practice before and you would like to re-register with us then you will need to come straight to the practice reception with your two forms of ID. We will then re-register you again straight away. Please don't fill in the online pre-registration form.

Over 75

From April 2014, all patients aged 75 and over will have a named GP who will have overall responsibility for the care and support that our surgery provides to them.
